Do you accept insurance?

No, we do not accept insurance. However, we do provide Super Bills that you can give to your insurance company for potential reimbursement, depending on your plan.

What are the benefits of having a membership?

Our memberships provide you with up to 30% off of services, up to 20% off of packages, urgent care visits, and even discounted labs and supplements! This is best for those committed to ongoing wellness care.

Are lab costs included in the price?

No, labs are not included. However, those who purchase a package or membership will have access to discounted labs. 

When are appointment payments due?

A $50 deposit is required to book appointment, which will be deducted from total cost. The remaining balance will be due at the time of service.

What is a Super Bill?

A Super Bill is a type of invoice that includes the provider's information (name, address, NPI number), patient information, date of service, CPT codes for services provided, diagnosis codes (ICD-10), and the fee charged for each service.

How will I receive my Super Bill?

You will typically receive your Super Bill immediately after your appointment. If you don't receive it, please contact our office and we'll make sure you get it promptly.

How do I submit my Super Bill to my insurance company?

Most insurance companies allow you to submit claims online through their member portal, via mail, or through their mobile app. We recommend contacting your insurance provider directly to determine the best submission method for your plan.

What should I expect during my first visit?

During your first appointment, you'll meet with our Nutrition Counselor who will ask questions about your diet, daily habits, and overall lifestyle to establish a baseline for your health. Then, you'll meet with Dr. Thomas who will review your medical history, current concerns, and work with you to develop a personalized treatment plan.

How do I schedule an appointment?
